Information You Provide

You warrant that all information provided to Origin HRT is accurate, complete, and not misleading at the time it is provided.

Where information is supplied from third parties, you confirm that it has been lawfully obtained and has not been altered, falsified, or misrepresented.

You must not alter or interfere with any clinical documents, reports, or records generated through our services.

You agree to promptly notify us of any changes to your:

• Legal name
• Email address
• Postal address
• Telephone number
• Payment details relevant to active services

Electronic Storage of Medical Records

By using Origin HRT services, you consent to the secure storage and processing of your personal and medical data in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018.

Medical records are retained in line with applicable UK healthcare guidance and may be held for up to 30 years, depending on the nature of the record.

You may request access to your personal data in accordance with our Privacy Policy.

Blood Testing Services

Blood testing services are provided through accredited third-party laboratories and may include clinician-reviewed reports where clinically appropriate.

All testing is conducted via venous blood sampling only and may require attendance at a partner clinic or the use of a mobile phlebotomy provider, including (but not limited to) Goodbody or Inuvi.

Clinical interpretation of results is undertaken in accordance with recognised UK guidance where appropriate, including that issued by the British Menopause Society.

Origin HRT reserves the right to change laboratory or phlebotomy providers where necessary.

Blood test results are provided for clinical information purposes only and do not constitute a diagnosis or replace a full medical consultation or emergency medical care.

Eligibility

To use Origin HRT services, you must:

• Be at least 18 years of age
• Have the legal capacity to enter into a binding agreement

We do not provide services to individuals under the age of 18.

Changes to These Terms

We reserve the right to amend these Terms & Conditions at any time. Where changes are material, reasonable notice will be provided.

Continued use of our services constitutes acceptance of the updated Terms.

Medical Disclaimer

All content provided via our website, platforms, or communications is for general informational purposes only and does not constitute medical advice.

You must not rely on such content to:

• Diagnose medical conditions
• Initiate or alter treatment
• Replace consultation with a qualified healthcare professional

If you believe you are experiencing a medical emergency, you must seek immediate medical assistance.

Communications (Opt-In)

Where you have consented, we may send you service updates, educational materials, or newsletters.

Such communications do not replace professional medical advice and you may opt out at any time.

Acceptable Use

You agree not to:

• Use our services unlawfully or fraudulently
• Provide false or misleading information
• Share prescriptions, medications, or clinical materials
• Interfere with system security or functionality
• Upload malicious software or harmful content
• Make misleading or unsubstantiated health claims

We reserve the right to suspend or restrict access where misuse is identified.

Blood Testing, Third-Party Services and Appointments

9.1 Third-Party Testing Responsibility

Where blood tests are arranged outside of Origin HRT's managed services — including where you independently purchase test kits or use a phlebotomist outside of our partner network — Origin HRT accepts no liability for:

• Failed or rejected samples
• Laboratory processing issues
• Improper sample collection, handling, or transport

In such cases, tests must be repeated at your own cost.

9.2 Appointments and Cancellations

Appointments involving third-party providers may incur costs at the point of booking.

If you:

• Fail to attend a scheduled appointment, or
• Cancel with less than 48 hours' notice

The appointment will be treated as missed and:

• The full appointment fee will remain payable
• Any subsequent appointment must be rebooked and paid for in full

This applies unless otherwise agreed in writing.

Payments

Payments for services, blood tests, and subscriptions are processed securely via third-party payment providers. Origin HRT does not store or process payment card details directly.

Payment providers operate in accordance with applicable regulatory requirements and industry security standards, including PCI DSS.

By making a payment, you agree to the relevant payment provider's terms and conditions.

Payment providers may include:

• Stripe
• GoCardless
• Revolut
• Trust Payments

Subscriptions and Recurring Services

Certain Origin HRT services are provided on a subscription basis.

11.1 Recurring Billing

By enrolling in a subscription, you agree that payments will be collected on a recurring basis (e.g. monthly) using your chosen payment method.

11.2 Minimum Term

Subscriptions are subject to a minimum term of four (4) months from the date of commencement.

During this period, fees are payable in full and the subscription cannot be cancelled, except where required by law or expressly agreed by Origin HRT in writing.

11.3 Renewal and Cancellation

Following completion of the minimum term, subscriptions will continue on a rolling basis unless cancelled.

Cancellation:

• Must be requested via your account or by contacting us
• Will take effect at the end of the current billing period

11.4 Failed Payments

If a payment is not successfully collected, we reserve the right to:

• Suspend or restrict access to services, including prescriptions and clinical support
• Recover any outstanding balances

11.5 Changes to Fees

We reserve the right to amend subscription fees. Where changes are material, reasonable notice will be provided.

Prescriptions and Medication Supply

Prescriptions are issued at the sole discretion of a qualified clinician and are subject to clinical assessment, safety considerations, and ongoing monitoring requirements.

Origin HRT does not guarantee that a prescription will be issued following consultation or at any stage of treatment.

You acknowledge that:

• Prescribing decisions may change based on clinical findings, blood test results, or evolving medical history
• Failure to complete required monitoring (including blood tests) may result in prescriptions being withheld or discontinued
• Prescriptions are personal to you and must not be shared, transferred, or used by any other individual

Where medication is supplied:

• Availability may be subject to third-party pharmacy providers and supply chain factors
• Delays outside of our control may occur

Origin HRT accepts no responsibility for misuse of prescribed medication or for use outside of clinical guidance provided.

Complaints

Complaints are handled in accordance with our Complaints Policy.

Clinical complaints will be reviewed by appropriately qualified personnel.

Privacy Policy

Your use of Origin HRT services is subject to our Privacy Policy, which forms part of these Terms & Conditions.

Third-Party Rights

These Terms & Conditions do not confer any rights on third parties under the Contracts (Rights of Third Parties) Act 1999.

Governing Law

These Terms & Conditions are governed by the laws of England and Wales.

The courts of England and Wales shall have non-exclusive jurisdiction.

We aim to respond to written communications within 48 hours on working days.

Administrative services are provided by Origin Health Care Group Limited (Company No. 16914475).
